چکیده

Background. Limited data on the incidence and clinical characteristics of IRIS in neutropenic patients with IPA were available. Methods. During 6-year period, adult patients with neutropenia who met probable or proven IPA by the EORTC criteria were retrospectively enrolled. IRIS was defined as new appearance or worsening of radiologic pulmonary findings temporally related to neutrophil recovery with evidence of a decrease 50% in serum galactomannan index titers. Results. Of 150 patients, 35 (23%, 95% CI 17% 31%) developed IRIS during neutrophil recovery. IRIS was associated with shorter neutropenic period, controlled underlying hematologic disease, less immunosuppressant use, and voriconazole use (table). The 30and 90-day mortalities were lower in patients with IRIS than those with nonIRIS (table). In the subgroup analysis of 53 patients who had progressive disease during the neutrophil recovery, 35 (66%, 95% CI 53% -77%) were classified as IRIS group. The 30and 90-day mortalities was higher in patients with non-IRIS who had progressive disease during the neutrophil recovery than in those with IRIS (9% vs 39%, P = 0.02 and 31% vs 72%, P = 0.005, respectively).

Diagnostic value of the serum galactomannan and (1, 3)-β-D-glucan assays for invasive pulmonary aspergillosis in non-neutropenic patients.

OBJECTIVE Galactomannan (GM) and (1, 3)-β-D-glucan (BG) are considered useful seromarkers for the diagnosis of invasive pulmonary aspergillosis (IPA) in patients with neutropenia. However, there is still limited data on these seromarkers for testing non-neutropenic patients who are at the risk of IPA. The aim of this study was to evaluate the value of these two serum antigen assays for the earl...

Multiple cavities with halo sign in a case of invasive pulmonary aspergillosis during therapy for drug-induced hypersensitivity syndrome

A 67-year-old female with rheumatoid arthritis and asthma-chronic obstructive pulmonary disease overlap syndrome was admitted for drug-induced hypersensitivity syndrome (DIHS) caused by salazosulfapyridine. Human herpes virus 6 (HHV-6) variant B was strongly positive on peripheral blood. Multiple cavities with ground grass opacities rapidly emerged predominantly in the upper and middle lobes. S...

Paradoxical Immune Reconstitution Syndrome Presenting as Acute Respiratory Distress Syndrome in a Leukemia Patient during Neutrophil Recovery

Immune reconstitution inflammatory syndrome (IRIS) in the setting of antiretroviral therapy is well described, but it is not as common in non-HIV patients; here, we present a case of immune reconstitution inflammatory syndrome presenting as acute respiratory distress syndrome in a leukemia patient who had neutropenic fever and septic shock after high-dose cytarabine. During neutropenia recovery...

PCR-ELISA for the early diagnosis of invasive pulmonary aspergillus infection in neutropenic patients.

AIM To evaluate a newly developed aspergillus mitochondrial gene PCR-ELISA assay for the early diagnosis of invasive pulmonary aspergillosis (IPA) in neutropenic patients. METHODS The aspergillus mitochondrial gene was chosen for the amplification target for use with a solution hybridisation assay with colorimetric end stage detection in microtitre plate format (PCR-ELISA). The study group co...

Frequent false-positive results of Aspergillus latex agglutination test: transient Aspergillus antigenemia during neutropenia.

BACKGROUND Two serologic assays, Aspergillus latex agglutination testing (LA) and plasma (1-->3)-beta-D-glucan (BDG) measurement, are used when invasive pulmonary aspergillosis (IPA) is suspected. Despite the high specificity of these assays, false-positive results are frequent for neutropenic patients. This study was conducted to evaluate the efficacy of LA and BDG and to investigate the cause...
